Spend some QUALITY time away from the daily grind
The RNZCGP Quality Symposium in Wellington, February 12-13 is now only one month away. Register now at http://www.rnzcgp.org.nz/quality-symposium/ - full programme available on Wednesday 13 January. Featuring international speakers Nick Goodwin (Kings Fund, UK inquiry into the quality of general practice, Chair of the International Network for Integrated Care); Candace Imison (Kings Fund, Integrated family Health Centers, UK inquiry into the quality of general practice); and Chris Mitchell (President Royal Australian College of GPs) the symposium will be a major learning and decision making forum for improving clinical outcomes for patients in primary care.

Ministry of Health Message: Change to Flu 2010 Strategy  

The following is a message from the Ministry of Health:
Influenza vaccination for pandemic flu (H1N1) is planned to start in February for health care workers and some high risk groups. Based on the northern hemisphere experience, a resurgence of pandemic influenza (H1N1 09) seems likely in late March leading to a peak in late April/early May. The 2010 normal seasonal vaccination programme will include the pandemic strain but it may not be available until mid-late March at the earliest. Therefore the Ministry will offer vaccination with a monovalent pandemic strain to frontline health care workers and some high risk individuals starting in early February.   Delivering the H1N1 monovalent vaccine presents some challenges - how to encourage people to be vaccinated for a winter illness in late summer and making the best use of the vaccine which is in 10 dose vials that must be used within 3 hours. In order to avoid wastage, creative solutions such as dedicated clinics and/or mobile arrangements will be needed in primary care. The details of the local response are to be developed by DHBs, PHOs, local practices and local immunisation coordinators working together. The Ministry will send out a fax with more information in early January.  This will include details of target groups, payments, vaccine supply and timing. College congratulates New Year’s Honours recipients
The College congratulates two of its Distinguished Fellows awarded MNZM (Member of the New Zealand Order of Merit) in the 2010 New Year’s Honours: Dr Jacqueline Sherburd Te Makahi Allan, for services to Maori health; and Dr John Panton Musgrove, for services to community health.  We also congratulate other health sector recipients: Professor Mason Durie, Dr Peter Fisher, Dr Frederick Graham, Dr Robert Boas, Assoc Prof Vernon Harvey, Prof James Horne, Dr Gordon Kirk, Dr Hugh Spencer, Dr Daniel Devadhar, Prof Sitaleki ’Ata’ata Finau and Maj Peter Wayne Jacobs.

Kirwan award ceremony
The College’s Meritorious Service Award Presentation to John Kirwan for his essential work on the National Depression Initiative was a lovely way to finish the year. At an Auckland ceremony, the legendary winger paid tribute to New Zealand’s GPs: “You do the hard work.” Media coverage was enthusiastic. HDC's guidance on open disclosure policies revised
HDC’s guidelines now apply to a wider range of health and disability service providers, and reflect developments in the area of open disclosure. The revised guidelines are available on HDC’s website here.

Survey about ACC’s new sensitive claims guidelines: final opportunity
The College would like to hear from any members currently using the new ACC sensitive claims guidelines by 9am Monday. The College has welcomed ACC’s moves to follow evidence-based care guidelines for sensitive claims clients. The new treatment framework is in line with the recommendations of the Massey guidelines for the assessment and treatment of mental injury as a result of sexual assault and sexual abuse. Some groups and individuals have raised questions about the implementation of the guidelines and others about whether GPs are able to provide a DSM IV diagnoses in support of sensitive claims. We are currently working on some guidance for vocationally registered GPs with respect to using DSM IV and will circulate this as soon as possible. Meanwhile, if you have not had the chance to do so, please complete the following brief questionnaire on your experience of using the guidelines to date. MOH Draft Guidance Paper: primary mental health care in the new primary care environment
The MOH have asked the RNZCGP for comment on this paper which outlines 13 key areas of focus to move towards optimal primary mental health care in New Zealand's new primary care environment.  It recommends that the primary mental health sector adopt a stepped care approach to service delivery. The document is designed to: encourage and disseminate good practice in primary mental health care; provide guidance on more cost-effective models of care to assist DHBs to make best use of existing funding and inform future planning where funding does become available. Following feedback the document will revised by the MOH and sent to the Minister of Health to formalise its status as a guidance document. This is an important document and as it is not available on the web it has been emailed out to those on the College database who have indicated an interest in mental health. Review of the Auckland School of Medicine
The College has been invited by Auckland University to contribute feedback towards a routine review of the Auckland School of Medicine. The structure and curriculum of the MBChB programme are excluded from the Terms of Reference, except in any case where they cross other issues the Committee considers. However, the logistical and operational aspects of the School’s teaching contributions to the MBChB are within the Terms of Reference. Should you wish to suggest content for the RNZCGP response or require further details on the review, please contact policy@rnzcgp.org.nz prior to January 22.

Review of education standards for Bachelor of Nursing
The Nursing Council are reviewing the education standards for undergraduate preparation of nurses and are seeking input about the quality and preparation of nursing graduates to meet health care needs of the public of New Zealand. Access to high-cost, highly-specialised medicines in New Zealand
The three person panel Review of Access to High-Cost, Highly-Specialised Medicines in New Zealand has released a preliminary report intended to promote discussion and elicit feedback. Parliamentary Inquiry into how to improve completion rates of childhood immunisation
The terms of reference of the inquiry include: to define, and make recommendations, as to what methods could be applied at minimal cost to improve immunisation in New Zealand. Instructions for those wishing to make an individual response are available here. The College will be making a response. Members’ comments and copies of individual submissions are sought and if emailed to policy@rnzcgp.org.nz prior to January 27 will be able to be considered in the preparation of the College submission.

Defining the role of the GP
Dr Jo Scott-Jones, College rep on the then Primary Healthcare Advisory council (PHCAC), has put considerable thought into the issues around the role of the GP in current and future primary care teamwork. The result is a paper ‘Defining the Role of the GP in a primary health care team: The GP genome and phenotype 2009-2020’. Jo and the College are keen to hear feedback by the end of January to policy@rnzcgp.org.nz. Jo’s paper is available here.

Needs Assessment and Care Planning Process for Use in Pregnancy and Well Child Tamariki Ora Services
The Ministry of Health has asked for feedback on a discussion document called ‘Draft Needs Assessment and Care Planning Process for Use in Pregnancy and Well Child Tamariki Ora Services’. This document came out of a review of the Well Child Tamariki Ora Programme initiated in 2006. The development of an evidence-based needs assessment and family care planning process was one of the projects identified in the review.  Stakeholders indicated support for moving away from the current risk assessment model towards a needs assessment and family care plan model. The Ministry are asking for input on the Draft Needs Assessment and Care Plan Process by 15 January 2010 and the College is able to share the draft with members who are interested in providing feedback. Clinical Leader – GPEP1 and PG Rural
This is a varied and interesting senior position for the College.  The primary accountability is to provide clinical leadership for the RNZCGP’s post graduate rural programme and stage one of the vocational general practice education programme.  The Clinical Leader will contribute to the College’s strategic focus of a high performing, sustainable workforce for general practice and play an important part in the quality delivery of general practice education. Skin Cancer Courses

HealthCert and the University of Queensland, Australia, have released the dates for skin cancer certificate courses. Essential theory and practical aspects are covered in basic or advanced workshops. 2010 courses include Certificates in Primary Care Skin Cancer Medicine; Surgery; Advanced Dermoscopy and Dermal Imaging. The workshops are presented by highly qualified, experienced lecturers and include hands-on practical sessions. Local expert presenter, Dr Sharad Paul (NZ) will be conducting the Surgery course and an Advanced Surgery Workshop. Dr Paul covers highly relevant and concise information for GPs. 2010 courses start in Melbourne, 20-21 Feb with the Certificate in Primary Care Skin Cancer Medicine and Primary Care Skin Cancer Surgery. The Certificate courses are individual short courses or they can be part of the Master of Medicine (Skin Cancer Medicine) programme. Mindfulness Training for Clinicians

14–18 February, Te Moata Retreat Centre, Coromandel Peninsula. The importance of mindfulness in reducing levels of stress and in supporting the well-being of clinicians and patients has been shown in a number of trials. Mindfulness is a profoundly simple and accessible way of using attention and supports the capacity of clinicians to cope with the everyday demands of practice and be compassionately engaged with the most difficult parts of themselves and their patients. James Carmody Ph.D is Associate Professor of Medicine at University of Massachusetts Medical School. He teaches and researches effects of mindfulness training on symptoms, immune function and well-being and is principal investigator on several NIH-funded clinical trials. He is a New Zealander and has practiced in mindfulness-based traditions for forty years and was an instructor in the UMass Center for Mindfulness Stress Reduction program. Tuition: $900. The tool introduces ABC, the new approach for managing people who smoke (as outlined in the New Zealand Smoking Cessation Guidelines), and contains practical information about giving brief advice and Nicotine Replacement Therapy.  The tool takes between 20 and 40 minutes to complete.

Network conference 2010
New Zealand’s premiere rural general practice conference – the New Zealand Rural General Practice Network conference - will be held in Christchurch next year at the city’s Convention Centre from March 11 – 14. It’s worth noting that the weekend of the conference coincides with the Ellerslie Flower Show in the garden city on March 10-14.
