Onsite visit exemption request form
The Foundation Standard requires an in-person practice visit. However, most recently with COVID-19 those visits haven’t always been possible, which has meant creating the option of a virtual visit.
If your practice is having difficulty completing the in-person assessor site visit, the College recommends that you first speak to your PHO. An extension may be granted in instances where it’s not safe to be assessed in person.
The College will consider these requests on a case-by-case basis and will only consider requests from practices. Certification will not be awarded to practices based on virtual visits when they were not approved by the College prior to the virtual site taking place.
