Health Practitioners can use the Police form linked below if they would like to report concerns related to patient firearms access. Health Practitioners can also use the form to report that they have received notification of a Firearms Licence holder who is not their patient.
The Arms Legislation Act 2020 requires Health Practitioners to consider notifying Police of health concerns related to their patient's firearms access.
