Are you looking for mandatory criteria that you haven’t completed yet?

In the example below the practice is in Year 3. Indicator 30 is assigned to Year 4 and Indicator 31 is assigned to Year 2. However, they both have zero percent signs in the completion column. This is a clue that there are mandatory criteria that need to be completed in the current year.

Once you open the indicator you will see that the mandatory criterion also has a pink background.

Have you recently completed a CORNERSTONE® cycle?

Once you’ve paid your fees, your new cycle will be available in QA2QI. Remember to click the “Start New CORNERSTONE® Self Assessment” button to begin.

How do I check my user name to access my practice's assessment?

If you have forgotten your user name and/or don’t know what your user name is, please contact the College at quality@rnzcgp.org.nz or you can phone (04) 496 5999 during normal office hours, 9.00am – 5.00pm (Monday to Friday).

How do I reset my log-in password?

1. Click on the 'Forgot your password?' button

2. After clicking this button, you will be brought to another page where you will need to enter your user name and then click 'continue' to proceed with the password resetting process.

After this, QA2QI will send you an email with a link back to select a new password. This email will be sent to the email that is registered in QA2QI for your username.

How do I access my practice's self assessment?

1. Login here. You can also access the QA2QI login page from the RNZCGP website under the "I'm a Practice" tab.

2. Once you have entered your login details, the screen will open to your Assessments. If you have not started your Self Assessment yet you will have to click 'Start New CORNERSTONE® Self Assessment', click this button once only and give the system time to load and create the assessment. Note, only users assigned the status of 'Practice Administrator' in QA2QI will see the option to start a new assessment. Only one person per practice is designated in QA2QI as the Practice Administrator. Check with your PHO or the College if unsure.

3. If you have started your Self Assessment then you will see your Assessement in progress. To access your assessment you need to click on your practice's assessment number.

4. Clicking on your practice's assessment number will open up your Programme Assessment. The screen will look different depending on whether you are completing your initial Entry CORNERSTONE® assessment of the Annual Programme.




How do I check the progress of my practice's self assessment?

1. Once you have accessed your assessment, you should see a bar showing the Overall Completion of the self assessment, see below. Note: if you are on the CORNERSTONE® Annual Programme, the bar is called 'Completion of the Year' and will not show until the required number of indicators for the current year are allocated. 

2. Each indicator has its own progress bar showing progress of the criteria within the specific indicator. If an indicator has been completed it will show 100% in a green bar.

3. If an indicator has not been completed, then it will either show 0%, or a partially completed indicator will have an orange bar showing the percentage completed.

A partially completed indicator will have an orange bar showing the percentage completed.



4. To open an indicator and access the assessment information, click on the arrow next to the relevant indicator.

5. An alternative is to click the 'Expand all' button which will open up all of the indicators in that section.



To collapse all indicators, click the arrow next to expand all and then click 'Collapse All' which will close all of the indicators in that section.

How do I add comments for my practice's self assessment?

1. To open an indicator and access the assessment information, click on the arrow next to the relevant indicator.

2. An alternative is to click the 'Expand all' button which will open up all of the indicators in that section.

To collapse all Indicators, click the arrow next to expand all and then click "Collapse All" which will close all of the indicators in that section.

3. After opening the indicator, you will see yellow self assessment boxes to enter comments into. TIP: The comment box can be expanded by clicking and dragging the bottom corner of the box, see example below with the original box and then the expanded box. This option can only be carried out when using Google Chrome or Mozilla Firefox.



4. Once a comment, and/or any evidence has been uploaded, the self assessment also needs to be set to 'Met', or 'Partially Met' from the drop down list in the 'Self Assessment' column in order for evidence to be submitted. Setting it to 'Not Met' will not allow the indicator to reach 100%.

5. The completion bar should fill upon completion of self assessment for the criterion. Note, for the criterion to be completed and the completion bar to fill, there must be something entered into the Comments box and/or some evidence uploaded.

Don't forget to regularly save your assessment activity!

How do I upload evidence for my practice's self assessment?

1. To open an idicator and access the assessment information, click on the arrow next to the relevant indicator.

2. An alternative is to click the "Expand all" button which will open up all of the indicators in that section.

To collapse all Indicators, click the arrow next to expand all and then click "Collapse All" which will close all of the indicators in that section.

3. To upload evidence for the criterion in the Indicator, click the paper clip symbol to the left of the criterion.

4. A window will open where you can choose to 'Upload new evidence' or 'Use existing evidence'. Initially it is likely you will need to upload new evidence until your library of evidence starts to build up.


5. To upload new evidence, click the button and another window will open where you can upload the evidence.

i. Click the 'Upload' button to find your proposed attachment from your local drive.
ii. Give the file a name, and wait until the upload button indicates the upload was successful.
iii. After it has uploaded, click the 'Add Evidence to Criterion' button.

6. Repeat this step as necessary to load your evidence. After new evidence has been uploaded, the evidence will be listed, see example below.


7. To use existing evidence that has previously been downloaded in QA2QI, click the 'Use existing evidence' button. A window will open listing evidence loaded previously.

 

8. To find relevant evidence there is a search box to search. Enter the name of the evidence.

9. Click the check box next to the relevant evidence and then click 'Use Selected Evidence'

10. To complete the upload of evidence for the relevant criterion, it must be saved.

11. The evidence will then be listed in the 'Evidence' column

12. Once a comment, and/or any evidence has been uploaded, the self assessment also needs to be set to 'Met', or 'Partially Met' from the drop down list in the 'Self Assessment' column in order for evidence to be submitted. Setting it to 'Not Met' will not allow the indicator to reach 100%.

How do I submit an Entry Programme self assessment to the assessors?

1. You can only submit your self assessment once the 'Overall Completion' bar reaches 100%. Once the year is at 100%, a 'Submit to Assessor' button will open up at the top of the page, see below. Make sure you have saved your work before you submit your assessment. Note, this button is only visible to users who are assigned the status of 'Practice Administrator' in QA2QI.


2. After you click the 'Submit to Assessor' button, the self assessment will be unable to edited further. 

Annual 1: How do I allocate the required number of indicators each year?

Note: You are required to allocate 10 indicators each year, unless your practice is satisfying standards under the 2011-2014 CORNERSTONE® programme then you are only required to allocate 9 indicators per year.

1. All indicators will say 'To be decided' until they have been assigned to a year. To assign to a year, there is a drown down list for each indicator. Click on the 'To be decided' drop box and select the year. 

2. You will be shown whether you have allocated sufficient indicators to be self assessed in the current year. (Year 1 in this example). 

3. After allocating an indicator to Year 1, this will update - see example below. You are required to allocate 9 or 10 indicators to each year.

4. After allocating 9 or 10 indicators to the current year, you can allocate other indicators to any other year if you wish, however these indicators will not be able to be accessed again until the current year has been submitted. 

Annual 2: How do I identify mandatory criteria?

1. After allocating 10 indicators to the year, most of the indicators will state "No criteria allocated to the current year" in the Completion column. These indicators will be satisfied in other years.

2. However, some of the Indicators are not allocated to the current year will still have a 0% in the Completion column. This is because these Indicators contain mondatory critera, for example, Indicator 12 contains criterion 12.2, that is mandatory.

3. When opening such indicators, you will be able to identify the mandatory criteria as these will be the only criteria able to be completed, with comments able to be made and/or evidence able to be uploaded.

Don't forget to regularly save your assessment activity!

Annual 3 : How to submit an Annual Programme self assessment for the year

1. You can only submit a self assessment for the year once the progress bar labelled 'Completion of Year (%)' reaches 100% - see example below where the completion bar is only showing 6% completion. 100% will only be reached when the allocated 10 Indicators (or 9 indicator for practices completing under the 2011-2014 standards), and all mandatory criteria have been completed. 

2. Once the year is at 100% completion, a 'Submit Year to 'Assessor' button will be open up at the top of the page. Please note that this button is only visible to users who have been assigned Practice Administrator status, in QA2QI. 

3. When you click on this button the current year's assessment activity will close and cannot be edited further. The next year will open and be able to be completed. Note that you do not need to wait for feedback from an Assessor before allocating and completing Indicators for the nest year.
