General practices with Equity accreditation have shown their commitment to quality by driving initiatives which have improved patient outcomes, advanced health equity and the patient’s experience.

Strengthening health equity in general practice

Every three years, general practices renew their commitment to health equity through the reaccreditation process. To support this journey, the College has produced an Equity reaccreditation module. This module is designed to help practices deepen their health equity focus and culture and continue improving health outcomes for all patients and whānau.

This new module has a focus on outcomes-based evidence and reflective practice, providing the opportunity for practices to show how they have embedded equity into their everyday work. The module retains its original four indicators but differs in how practices showcase their progress and achievements.

We enjoyed showcasing our hard work and innovative approaches to care when working with patients/whanau in the equity space.

About the reaccreditation module

The reaccreditation module is structured around the four indicators, retained from the original Equity module. The reaccreditation module has a stronger emphasis on reflection and outcomes, moving beyond the planning and intent of the first module.

Included are:

  • Flexible, outcomes based evidence requirements
  • A new focus on reflective practice and learning
  • A shift from showing plans to demonstrating real progress
  • A set of forms designed to help your practice efficiently demonstrate evidence, understand what to provide, and how best to provide it.

Being able to reflect on where we have improved over the last 3 years is rewarding.

Why complete the module?

Completing the Equity reaccreditation module reaffirms your practice’s ongoing commitment to health equity, strengthens relationships with the communities you serve, and contributes to a health system where everyone has a fair opportunity for better health.

Cornerstone-accredited and teaching practices can maintain their accreditation by successfully completing both the CQI and Equity Reaccreditation modules.
