Coordinated palliative care

Well-coordinated palliative and end-of-life care provides people and their family/whānau with humane and dignified support and services while they face their life-limiting condition.

This care is essential. Patients with palliative care needs must be able to access their doctor or an informed deputy at all times. The practice must work out how these patients can contact their GP and their designated representative within working hours and out of hours.

Make sure this information is provided to the patient and family/ whānau. If a deputy is used, this delegated representative must be well informed about the patient’s condition and care.
